Agency Certification Forms

An Agency Certification From (AC) is a form that has to be filled out by the students Gain/Cal-Works worker. The form is used by C.A.R.E. to verify that the student is single, head of house hold, is receiving cash aid, and has a child under the age of 14. Agency Certifications must be turned in by the deadline each semester in order to be eligible. For specific deadline information, or if you have questions please fee to free to call the E.O.P.S.
