We strongly recommend all students start by downloading and completing the Certificate Planner Sheet. You may find that you qualify for one or more certificates. You may also find that you’re only one or few classes short for an additional certificate. Use this Certificate Planner Sheet to verify which certificate(s) you’re qualified for and/or as a guide to plan for your next certificate(s).

Once you have determined which certificate(s) you’re qualified for, proceed to click on the link below to download the corresponding certificate application form. Each certificate application form is partly prefilled for your convenience. Student needs only to complete the remaining information, sign and e-mail as an attachment to Silvia Serrano (@email) along with your unofficial transcript (making sure all your grades appear on your transcript).
