DSP&S Requests and Forms
The following requests and services are available only to students registered with East Los Angeles College DSP&S and are currently enrolled in courses. Services are not available for waitlisted courses or courses at other colleges.
Classroom Supports
Accommodation Letter(s)
Have DSP&S send your instructors a letter listing your approved classroom and testing accommodations (without sharing your diagnosis). Submit at the start of each semester or when you add a new class.
Shared Notes Program
If you are approved for notetaking assistance, DSP&S will send your instructor a memo to announce in class for a volunteer peer notetaker. Submit at the start of the semester or as soon as you need this support.
Adaptive Furniture & Preferential Seating
Arrange specialized seating or tables—such as height-adjustable desks or ergonomic chairs—in your classrooms. If you qualify for preferential seating, DSP&S can also provide a label to reserve your preferred desk or station. Submit as early as possible before the semester begins or when you notice a need.
ASL Interpreting & CART Services
Access American Sign Language (ASL) interpreting or Communication Access Realtime Translation (CART) for your classes or other DSPS services such as counseling and tutoring. Submit at least 2 weeks before the date you need the service.
Approval from DSP&S is required for a personal attendant or aide to accompany you to class. Both the student and personal attendant must attend an appointment with a DSPS disability specialist, bringing the completed Personal Attendant Agreement form, verification of employment or assignment, and valid identification. Approval is required each semester and whenever there is a staff change.

Testing Accommodations
DSP&S Test Proctoring Reservation (Student)
Reserve a seat with DSP&S to take your quizzes, midterms, or finals with approved accommodations. Submit at least 3 business days before your exam date.
DSP&S Instructor Test Proctoring Request
Technology & Accessible Materials
Equipment Loan & Software Request
Borrow assistive technology—such as smart pens, digital recorders, or other devices—to support your learning. Submit at the start of the semester or as soon as you identify a need.
Alternate Media
Get course materials in an accessible format—such as Braille, large print, audio, or digital text—to meet your needs. Submit as soon as you register for classes or receive your syllabus. Proof of ownership or rental is required,
Enrollment & Documentation
Reduced Unit Load Verification for EOP&S or LACP
Request verification from DSP&S confirming your eligibility to take fewer units due to a disability so you can maintain program benefits. Submit when proof is needed for EOPS, LACP, or another program.
Program Enrollment Verification & Records Request
Obtain DSP&S letters for situations outside of class accommodations, such as scholarships, program applications, or community resources. Submit whenever you need official DSPS documentation or a copy of your verification of disability
